The consents on file, and the forms printed from them.
The printed form carries no clauses.
Whether CBS supplies statutory consent wording is decision D7 and it is
open, so the form prints the facility's letterhead, the consent on record
and an explicit statement that the wording was not supplied. Add the
hospital's own text before anybody signs one.
